North Beverley PCN

As a result of changes in the GP Contract, practices are now working together in groups - typically covering between 30-50 thousand patients. We are a member of the NorthBeverley Primary Care Network

Primary Care Networks (PCNs) build upon the core of previous primary care services and allow greater provision of proactive, personalised, co-ordinated and better integrated health and social care.

Primary Care Networks are the foundation for future collaborations with other providers such as Community Trusts where appropriate, and will work together to bring specific packages of care to improve the health of patients.

The Local Authority Health Profiles provide an overview of health for each local authority in England.   They pull together existing information in one place and contain data on a range of indicators for local populations, highlighting issues that can affect health in each locality.

Locally a Health Profile for each of the seven Primary Care Networks across the East Riding have been produced by the Public Health team at East Riding of Yorkshire Local Authority.
