Social Prescribing

Social prescribing enables GPs and other health professionals to refer patients to a range of non-clinical services through a link worker. This holistic approach of connecting people with community groups and other services enables practical and emotional support to be provided and helps people with their health and well being. It works for a wide range of people including those:

• with one or more long-term conditions
• who need support with their mental health
• who are lonely or isolated
• who have complex social needs which affect their wellbeing
